How to Register
To register with our practice please complete the following 3 steps:
1. Catchment Area
Check you are living within our catchment area, enter your postcode below and if you are within the area then proceed to step 2 or click the Register now button above.
Crown Street Surgery also gives the option to register for people who are not living within our practice catchment area, please click here for more details.
To register with our practice, please come into reception to get a form, or complete the registration form below. Alternatively you can register online, simply click the register online button below.
2. NHS Number
Click the button below to check your NHS number. This is needed for your registration, you can register without it but the process may take a little longer.

Registrations can take up to 15 working days to be processed. You will receive a text message confirmation once you are registered with us.
We invite all our new patients to see one of our practice nurses or our health care assistant. This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems. Normally, you will need to complete a registration medical before seeing one of our GPs.
Out of Area Registrations
We offer registration for patients who live outside our usual practice area. Here’s what you need to know:
– Voluntary service: GP practices do not have to accept out‑of‑area patients.
– Home visits: If you register with us but live outside our area, we are not required to provide home visits. Instead, you should call NHS 111, who will arrange local support or a home visit if needed.
– Registration decisions:
We may refuse your registration if we do not have capacity, or if it is not clinically appropriate or practical for you to be registered so far from home.
Our GP partners will review your form and decide if we can accept you without home visiting duties.
This process can take 7–10 days.
– Future changes: If your circumstances change, you may be asked to re‑register with a GP closer to home.
– If you are too ill to attend: Call NHS 111. They will advise on local services or arrange a home visit.
– Impact on referrals: Being registered away from home may affect hospital referrals, tests, or access to community health services. Please discuss this with your GP.

Overseas Visitors
Registering with a GP practice ensures that any care provided by the GP is free of charge. However, this does not automatically mean you are entitled to free NHS hospital treatment.
You may also be invited for NHS screening services as part of your registration, but some services not provided by the GP may require payment.

Temporary Residents
If you are in England for less than 24 hours and need to see a doctor you can be registered as needing ‘immediate and necessary treatment’. This means you can receive emergency treatment for up to 14 days. After 14 days you will need to register as a temporary or permanent patient.
