After you have completed and submitted the form you will be contacted within 2 working days after which you will then be requested to attend the surgery within 7 days with 2 forms of identification (ID).
Patients may be required to provide some information when registering should the practice need to confirm the spelling of their name, current name ( if this has been changed in the past) or accurate date of birth. We will request you provide ID to confirm these details if necessary.
After you have completed and submitted the form you must attend the surgery within 7 days to complete or book in your new patient health check.
We invite all our new patients to see one of our health care assistants or trained staff. This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems. Normally, you will need to complete a registration medical before seeing one of our GPs.
Until you have attended your new patient check you will not be registered at the practice.
How do I register as a temporary resident with a GP?
The surgery will ask you to complete a Temporary services form GMS3 (PDF, 81kb).
You can register as a temporary resident if you plan to live near the GP surgery for up to 3 months.
